ā€œPatients can be quite disappointed when the reality of their changed function is discussed,ā€ Dr Afari explained. ā€œThis disappointment can erode their trust in the healthcare team and diminish their engagement with treatment.ā€

Dr Afari recounted a case involving a middle-aged woman who had undergone a below-knee amputation. Shortly after surgery, he explained her post-operative care plan, emphasising the need for inpatient rehabilitation to adapt to using a wheelchair while awaiting a prosthesis.

However, the patient was both surprised and upset. She had been told by her surgeon that she would be fitted with a prosthetic leg within 6-8 weeks and would remain an inpatient until that timeā€”a timeline that was unfeasible given her co-morbidities. Adding to her distress, she had purchased a smaller home in anticipation of walking into it with her new prosthetic.

ā€œThe patient was already grappling with the grief and pain of losing a limb,ā€ Dr Afari said. ā€œThe sudden addition of new, contradictory information compounded her distress.ā€

This case highlights the importance of providing accurate, realistic expectations at the earliest stages of care.

ā€œWe need to explain all possible outcomes, the emotions and sensations patients might experience, and what to expect from other health professionals involved in their care,ā€ Dr Afari said.

Healthcare providers should also take the time to understand the roles and limitations of other practitioners involved in the patientā€™s treatment. Miscommunication between care providers can inadvertently result in promises that others cannot fulfil, creating confusion and disappointment for the patient.
